Our Team is YOUR Team
Tender Care represents an entire team of health care professionals whose purpose is to help you.

Physicians - Our Medical Director works with your personal physician to build a comprehensive plan to control pain and other symptoms of terminal illness.

Nurses - Our trained and experienced nurses will develop and manage a personalized plan of care just for you. We focus on your individual needs and provide your entire family with compassionate, tender care.

Certified Nurse Assistants - Some of the most valuable members of our team include well trained professionals who will provide you with the personal care and the help you need for daily living. They also offer companionship and valuable emotional support.

Other Services - You and your family will become part of a team of Social Workers, Bereavement Counselors, Chaplains, and Volunteers. Our team is YOUR team.

Who is Eligible for our Services?
Hospice care can be elected by individuals who have been diagnosed with a terminal/life-limiting illness. The patient's attending physician must certify that the patient has a life expectancy of six months or less if the illness runs its normal course.

Hospice is a 100% covered benefit of the Medicare Part A program and is also covered by Medicaid. Most private insurance companies, HMOs, and managed care plans also offer hospice care as a benefit.

Also, please do not let the "six month" life expectancy window trouble you. There is NO time limit to the hospice benefit.
